Not all of our counselors can bill all accepted insurance plans listed above. Please contact your insurance agency to find approved providers, or call our office to check on the availability of counselors able to bill your plan.
If we are not able to bill your insurance company for our services, we offer a Low Cost Plan for self-pay clients. Our fees are based on a scale that considers the income and family size. To find out what your session fee would be, please contact our office.
No Surprises Act
As of January 1, 2022, all health care providers are required to provide a “Good Faith Estimate” (GFE) to any person who is receiving services, and who is not using insurance or doesn’t have insurance for those services.
Important information on this law and how it affects you when receiving counseling at CCMM.
The No Surprises Act was created to avoid consumers receiving a “surprise” bill from an out-of-network provider. It is intended to make it easier to understand all that you will be billed.
You have the right to receive a GFE for the total expected cost of any non-emergency items or services you are receiving from CCMM.
CCMM will provide you with a GFE in writing no later than 1 business day after the date on which you scheduled your initial counseling appointment. You can also ask your health care provider for a GFE before you schedule an appointment.
If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
You should always keep a copy or picture of your current GFE.
In the GFE we will provide you with, we are expected to include: the services requested, potential diagnoses, the name of the provider(s), the frequency of those services, and an estimate of the total cost for each appointment.
Your GFE can be reviewed at any time upon your request, and your counselor may also initiate a review of your estimate if necessary.
The GFE is based on our service charge rate and adjusted according to your household size and income. It could go up or down as a result of a change in any of those facts. If a change does occur, a new GFE will be provided to you.
